Al-Thaqalain Oncology Hospital in Basra, affiliated with the Health and Medical Education Authority of the Imam Hussain Holy Shrine, has announced the successful completion of a complex surgical procedure to remove a massively enlarged multinodular thyroid gland without the need to split the breastbone or open the rib cage.
Speaking to the official website, oncology surgeon Dr. Mohammed Taher said that "Al-Thaqalain Oncology Hospital in Basra, affiliated with the Health and Medical Education Authority of the Imam Hussain Holy Shrine, successfully performed a complex surgical procedure to remove a massively enlarged multinodular thyroid gland that had extended behind the sternum and into the chest cavity, without the need to split the breastbone or open the rib cage, for a patient in his mid-fifties."
He explained that "the patient had been suffering from chronic enlargement that had been left untreated for several years and had previously affected him only cosmetically." He noted that "the condition recently progressed and symptoms began to appear due to pressure from the enlarged gland and nodules on surrounding tissues and organs, causing severe difficulty in swallowing and breathing, in addition to a noticeable change in the tone of his voice."
He added that "the surgery involved three major challenges. The first was the enormous size of the gland and nodules, with the left lobe measuring nearly (10 cm) and the right lobe between (7–8) cm, while the connecting section measured approximately (4) cm, which is extremely large for the neck region. The second challenge was the extensive blood supply to the tumor, increasing the risk of bleeding. The third and most significant challenge was the extension of part of the left lobe and nodules behind the sternum into the chest cavity."
He continued that "after the case was reviewed by the multidisciplinary scientific committee, a decision was made to proceed with complete removal. The surgery was performed with high precision and bleeding was successfully controlled. The portion extending into the chest was completely removed through the same small conventional neck incision, without the need to open the chest." He pointed out that "the nerves supplying the vocal cords and the parathyroid glands responsible for regulating calcium levels were fully preserved. The patient regained his voice immediately upon waking, and normal calcium levels were confirmed."
He noted that "long-standing thyroid enlargements are often benign; however, surgical treatment remains necessary to prevent complications caused by pressure on the trachea and other vital organs." He added that "prolonged neglect may, in some limited cases, lead to cellular changes and activity resulting in malignant tumors, making early surgical intervention the preferred option."
